1517 RCMP Prediction of Therapeutic Success with OAT—Remmers et al INTRODUCTION Obstructive sleep apnea (OSA) poses a significant medical challenge to industrialized societies because it is common, carries cardiovascular and safety risks, and compromises quality of life.1-5 The predominant therapy for OSA is nasal continuous positive airway pressure (CPAP), a highly efficacious and benign treatment. However, adherence to this therapy is reported to be low, raising the need for alternative treatments.6,7 Surgical procedures, other than those involving bony reconstruction, have not proven to provide long-term benefit.8 Pharmaceutical and neuro-stimulatory approaches have been the subject of research, but no therapies in these categories are, as yet, generally available. Mandibular repositioning appliances (MRAs), which hold the mandible anteriorly and inferiorly during sleep, have been shown to open the passive pharynx REMOTELY CONTROLLED MANDIBULAR PROTRUSION IN OSA PATIENTS

Mandibular advancement and obstructive sleep apnoea: a method for determining effective mandibular protrusion.

The objectives of the study were to test the hypotheses that it is possible, during routine polysomnography (PSG), to prospectively identify favourable candidates for mandibular repositioning appliance (MRA) therapy in the treatment of obstructive sleep apnoea (OSA) and to accurately estimate an optimal protrusive distance at which to fabricate the MRA. A series of subjects underwent a remotely...

Mandibular advancement appliance for obstructive sleep apnoea: results of a randomised placebo controlled trial using parallel group design.

The aim of this trial was to evaluate the efficacy of a mandibular advancement appliance (MAA) for obstructive sleep apnoea (OSA). Ninety-three patients with OSA and a mean apnoea-hypopnoea index (AHI) of 34.7 were centrally randomised into three, parallel groups: (a) MAA; (b) mandibular non-advancement appliance (MNA); and (c) no intervention. The appliances were custom made, in one piece. The...

Can mandibular advancement devices be a satisfactory substitute for short term use in patients on nasal continuous positive airway pressure?

BACKGROUND Mandibular advancement devices (MADs) can successfully control both snoring and obstructive sleep apnoea (OSA). Many patients on nasal continuous positive airway pressure (NCPAP) for OSA would like a more portable alternative, even if only temporarily. This study assesses what proportion of patients with OSA already on NCPAP can successfully use a MAD for short periods (up to 1 month...

Effects of mandibular posture on obstructive sleep apnea severity and the temporomandibular joint in patients fitted with an oral appliance.

STUDY OBJECTIVES The purposes of this study were to assess the relationship between different increments of mandibular protrusion and a reduction of obstructive sleep apnea (OSA) severity and to use magnetic resonance imaging to assess temporomandibular joint (TMJ) morphology and condyle position after the insertion of an oral appliance (OA). DESIGN Six mild to moderate OSA patients were trea...

Comparison of Titratable Oral Appliance and Mandibular Advancement Splint in the Treatment of Patients with Obstructive Sleep Apnea

Objective. To compare the effect of two intraoral devices (titratable oral appliance-Klearway (KW) and mandibular advancement splint (MAS)) in mild and moderate obstructive sleep apnea (OSA) patients. Method and Materials. The study group was comprised of twenty-four adult volunteer patients with OSA. Twelve subjects were fitted with a titratable oral appliance (group KW) protruding the mandibl...
